Brow Lift vs. Upper Blepharoplasty

Have you noticed that your eyes look more tired, heavy, or less open than they once did? Perhaps you've found yourself looking permanently exhausted or have been told you appear worried, even when you feel perfectly well.

Changes around the eyes are a natural part of ageing, but the cause isn't always obvious. For some people, the eyebrows gradually descend, creating a heavier appearance across the upper face. For others, the problem lies in excess skin on the upper eyelids. Because both conditions can produce similar symptoms, it's common to confuse a brow lift with upper blepharoplasty (upper eyelid surgery).

Although both procedures are designed to rejuvenate the upper face, they address different concerns. Understanding the difference is the first step towards choosing the treatment that's right for you.

What's the Difference Between a Brow Lift and Upper Blepharoplasty?

The forehead, eyebrows, and eyelids work together to frame the eyes and play an important role in facial expression. As we age, collagen and elastin levels naturally decline, causing the skin and underlying tissues to lose firmness. This can lead to drooping eyebrows, hooded eyelids, or, in many cases, a combination of both.

A brow lift focuses on repositioning the eyebrows to a more youthful height. By lifting the brow, the procedure can open up the eye area, reduce heaviness, and create a brighter, more refreshed appearance. Depending on the surgical technique used, it may also soften forehead lines and improve the overall balance of the upper face.

Upper blepharoplasty, on the other hand, treats the eyelids themselves. The procedure removes excess skin and, where appropriate, small amounts of fat from the upper eyelids. It is often recommended for people with hooded eyelids or loose skin that makes the eyes appear tired or, in some cases, affects the upper field of vision.

While the two procedures can produce similar aesthetic improvements, they treat different anatomical structures.

Brow Lift

Upper Blepharoplasty

  • Lifts the eyebrows

  • Treats drooping brows

  • Opens and refreshes the upper face

  • May reduce forehead heaviness

  • Removes excess upper eyelid skin

  • Treats hooded eyelids

  • Improves the appearance of the upper eyelids

  • May improve vision if eyelid skin obstructs it

Which Procedure Might Be Right for You?

The answer depends on what's causing the heaviness around your eyes rather than how your eyes look alone.

If your eyebrows have gradually lowered over time, particularly towards the outer corners, a brow lift may provide the most natural improvement. Some people also find themselves unconsciously raising their eyebrows throughout the day to compensate for the heaviness, which can contribute to forehead lines over time.

If your eyebrows remain in a good position but loose skin folds over the upper eyelids, upper blepharoplasty may be the more appropriate option. Removing the excess skin can create a fresher appearance while preserving your natural facial expression.

Many patients experience a combination of both concerns, which is why it can be difficult to determine the most suitable treatment simply by looking in the mirror. A consultation with a consultant oculoplastic surgeon allows your facial anatomy to be assessed properly before any recommendations are made.

Can a Brow Lift and Upper Blepharoplasty Be Performed Together?

Yes. In many cases, the two procedures complement one another.

Ageing rarely affects just one area of the upper face. It's common for the eyebrows to descend while excess skin also develops on the upper eyelids. Treating only one concern may not always provide the most balanced or natural-looking result.

Where appropriate, our consultant may recommend combining a brow lift with upper blepharoplasty to address both issues during the same operation. This approach can enhance the overall appearance of the upper face while allowing patients to recover from both procedures at the same time.

The decision will always depend on your individual anatomy, your concerns, and the outcome you're hoping to achieve.
